Provide skilled care to patients in their residence under the direction of a RN and in accordance with the physician plan of care. To ensure safe and quality delivery of home health physical therapy services and to assist in the provision of home health physical therapy services that reflects the agency philosophy and standards of care to assigned patients under the direction of a physical therapist. - Registered Nurse
Manages patient case load assigned in accordance with agency policy and acceptable standards of practice. Makes the initial evaluation visit, regularly reevaluates the patient’s nursing needs when there is a significant health status change in the client’s condition, at the physician’s request, or after hospital discharge. Initiates the plan of care and necessary revisions and the appropriate preventive and rehabilitative nursing procedures to achieve patient goals.
