NOTICE ON EMPLOYABILITY INVESTIGATIONS

Before continuing with this application, you are required to understand that it is Angelitos Health Care, Inc. d/b/a Angelitos Preventive Health Care's ("Angelitos") policy to perform an employability investigation on all individuals seeking employment with Angelitos. You are also required to consent to such an investigation. If you are applying for a position that may qualify you as a controlling person, a person with a disclosable interest, an agency affiliate, the agency's administrator, the agency's alternate administrator, or the chief financial officer of the agency you must not have been convicted of an offense described in TAC, Title 40, Part I, Chapter 99 (relating to Criminal Convictions Barring Facility Licensure).

If your duties would include face-to-face contact with a client and your criminal history check includes a conviction listed in THSC, Title 4, Subtitle B, Chapter 250.006 (relating to Convictions Barring Employment for Employment Applicants in Certain Facilities Serving the Elderly, Persons with Disabilities, or Persons with Terminal Illness), you are automatically barred from employment with Angelitos. If you have a conviction that is not listed in THSC, Title 4, Subtitle B, Chapter 250.006, your conviction will be formally reviewed by the agency to assess whether it is a contraindication to employment.

Angelitos utilizes the Texas Department of Public Safety's Crime Records Service to acquire any information related to convictions. In the state of Texas, all DADS-regulated facilities and agencies are required to check both the Nurse Aide Registry and the Employee Misconduct Registry before hiring an individual to determine if the person is listed as having committed an act of abuse, neglect, exploitation misappropriation, or misconduct against a resident or consumer and is, therefore unemployable. If, after having submitted your application for employment, you are formally interviewed you will be contacted by a representative of Angelitos thereafter to obtain your date of birth in order to facilitate our use of the Crime Records Services. Furthermore, understand that at any point during the application process but always before hiring you or before your first face-to-face encounter with a client of Angelitos, the agency will utilize the Texas Department of Aging and Disability Services' Regulatory Services Credentialing Sanctions Database (http://www.dads.state.tx.us/providers/NF/credentialing/sanctions/) and the Texas Department of Aging and Disability Services' Employability Status Check website (http://www.dads.state.tx.us/providers/employability/esearch.cfm) or toll-free number (1-800-452-3934) to determine your eligibility for employment.
